ITC Healthy Start Nurse - Part Time

SUMMARY

Provide and coordinate Home Visiting project for the Tribal community. Coordinate comprehensive health care and social support services to prenatal and postpartum/Inter-conception women and their families living within the six-county service area and/or Tribal health service areas.

MINIMUM QUALIFICATIONS

  • N. licensure to practice in the State of Michigan required; Bachelors of Science in Nursing preferred.
  • Professional experience in community health, obstetrics and/or prenatal health care management preferred.
  • Must have a valid Michigan Driver's license and be insurable by the GTB insurance carrier.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide services to the Native American population and inform the community about maternal and child health, family advocacy and social services.
  • Conduct home visits to pregnant women and enrolled participants for recruitment and follow-up.
  • Perform psychosocial, physical, behavioral health, and nutritional screening and client counseling regarding health promotion/risk reduction activities.
  • Promote healthy child development through education and demonstration of positive parenting techniques.
  • Check if clients need anything or are having any problems.
  • Supply immunization reminders and monitor appointments.
  • Provide screening, education and referral services during pregnancy to include, but not limited to: smoking, alcohol, domestic violence, and substance abuse cessation and prevention education for Fetal Alcohol Syndrome (FAS).
  • Arrange and/or provide transportation for women to appointments as needed.
  • Use referral system to identify at-risk/high-risk pregnant women to enroll in prenatal care and other services.
  • Develop appropriate plan of care and referrals to other health care providers and serve as advocate for families needing referral and follow-up.
  • Establish close working relationships with Family Spirit, Head Start, Early Head Start, and Early On for cross-referrals, family services and sharing of educational materials.
  • Implement plan of care in conjunction with other Tribal and community health personnel, evaluate and refer high-risk mothers as needed.
  • Organize case management forms, files of participant records, ensure data is entered into online data system and compile quarterly reports.
  • Contribute to statewide project efforts to incorporate Native American culture and values into services, activities and materials.
  • Assist to resolve problems facing the Tribal community concerning housing, education, welfare, unemployment, crime prevention, substance abuse and other domestic issues
  • Attend case management, consortia and Healthy Start quarterly meetings, as well as additional training, networking conference, etc.
  • Plan, implement and report community education group events.
  • Maintain records of all MCH participant contacts and activities using project forms.
  • Establish and promote linkages with various programs, services and other local programming for children.
  • Provide appropriate documentation of care as specified by the Tribal community health nursing policies, and as outlined in Healthy Start Tribal sub-contract policies and procedures.
  • Conduct self in a professional and ethical manner.
  • Maintain confidentiality.
  • Maintain accurate, timely and confidential records.
  • Establish and maintain supportive work relationships.
  • Accurate and timely submission of records, reports, etc.
  • Respond to ITC information requests in a timely manner.
  • Other duties as assigned by supervisor.

  • Ability to work collaboratively with other areas and personnel within the Benedjenh Center.
  • Must have excellent health education and presentation skills.
  • Ability to quickly evaluate alternatives and decide on a plan of action.
  • Must possess the ability to listen and communicate effectively to patient needs and use good judgment in directions given to patients.
  • Uses good judgment in interpreting guidelines, in adapting department procedures, or in recommending approaches or solutions for specific problems.
  • Working knowledge of computers, preferably in Microsoft Windows, Word, Excel, Outlook, and other Microsoft office products and database applications.

EDUCATION and/or EXPERIENCE

  • N. licensure to practice in the State of Michigan required, B.S.N. preferred.
  • Professional experience in community health, obstetrics and/or prenatal health care management preferred.
  • Must be able to work with and respect cultural and racial differences.
  • Working knowledge of community resources related to health and human services, maternal and child health and family support. Working knowledge of general office and clerical practices.
  • Excellent interpersonal skills required. Strong computer skills necessary, including word processing and electronic file management.

OTHER QUALIFICATIONS

  • Must have valid Michigan Driver's license and be insurable by the GTB insurance carrier. Must be able to travel.
  • Previous experience working with Native American organizations preferred.

SUPERVISORY RESPONSIBILITIES

No supervisory responsibilities

EQUIPMENT TO BE USED

General office equipment such as, calculators, computers, modem, copiers, fax, phone systems, etc., and other clinic equipment required to perform necessary tasks.

TYPICAL PHYSICAL DEMANDS

Work requires sitting, lifting, reaching, walking, and lifting heavy objects. Must be in good physical and mental health. Lifting of patients may be required in assisting them through an examination. This position may at times have to assist patients in dressing/undressing.

TYPICAL MENTAL DEMANDS

The employee uses judgment in identifying and selecting the most appropriate procedures to use, or in determining which of several established alternatives to use. Some deadlines are involved, so there is time pressure on occasion.

WORKING CONDITIONS

General office environment and frequent in-home visits to families. Frequent travel required, including transporting clients in personal vehicle.

COMMENTS

Native American preference will apply. Must be willing and able to pass a background investigation and a drug and alcohol urinalysis as a condition of employment. Adherence to strict company policy regarding confidentiality is a must.
